What to Bring if You’re Applying for Health Coverage
For all programs, you will need to provide:
- Proof of income (pay stub, unemployment insurance stub, income tax return, Social Security award letter, a letter from your employer if you are paid in cash*)
- Government-issued Photo ID (State ID, Driver License, Passport, Military ID card, etc.)
- Proof of address (such as a utility bill with your name on it, or a driver license*
- If available, you can additionally provide your:
- Birth certificate
- US residency card (if applicable)
- Social Security card (if applicable)
- Proof of pregnancy (for Medi-Cal and Fam PACT)
Do you qualify for Medi-Cal?
Thanks to the Affordable Care Act, more people are eligible than ever before. You may qualify if:
- Your annual income is at or below $22,025 for an individual (amounts vary by family size)
- You are pregnant
- You are blind or disabled
- You are already enrolled in Cal-Fresh, Cal-WORKs, SSI/SSP, Refugee Assistance, or Foster Care/Adoption Assistance

Is your Medi-Cal due for renewal?
Medi-Cal renews once a year. If you’ve received a renewal packet, it’s time to act.
You have 90 days to complete your renewal — if you miss the deadline, you could lose your coverage. For those in a restricted immigration category, you may not be able to reapply.
